Issue Bottom of footing elevations did not leave adequate room for Contractor's formwork. Action Item Structures Management will consider revising Design Manual to recommend detailing bottom of footings a minimum of 3 feet above mean high water elevation or the ground elevation. Issue Drilled shaft permanent casing elevations, setting elevations vs. reviewing elevations. Action Item Geotechnical will review policy for determining drilled shaft permanent casing elevations. Issue Contractor noted substructure geometry was not consistent and resulted in less efficient construction. Action Item Structures Management will consider revising Design Manual to recommend detailing similar substructure configurations where practical. Issue Girders were over cambered and created issues with maintaining grade. Action Item Structures Management will review projects with similar girder types and lengths and investigate if those girders experienced camber issues. Issue Contractor experienced challenges with casting the bridge deck because interior barrier rail bars prevented the screed carriage from finishing the entire width of the deck. Action Item Structures Management and Construction will investigate and discuss details for casting bridge decks with interior barrier rails. Issue Additional easement had to be acquired for temporary work bridge. Action Item Roadway and Right of Way will ensure temporary work bridge is accounted for during property acquisition for future projects. |
